How should I prepare for my first appointment?
Discovery Sessions:
No paperwork needed! Just arrive 5 minutes early with your questions ready for our specialist.
Physical Therapy Evaluation:
Please check your email for intake forms and information on what to expect. Print and complete the forms, and wear loose, comfortable clothing that allows easy access to the area needing treatment.
Women’s Health Evaluation:
Check your email for intake paperwork and details. Print and bring the completed forms with you. Wear leggings and a loose, comfortable shirt for your appointment.
For ALL Sessions:
Don’t forget to bring your calendar! Our slots fill up fast, and booking your next appointment immediately is recommended. Most patients schedule a month in advance to secure their preferred times.
Can I bring someone to the appointment?
Absolutely! Feel free to bring along someone for support during your appointment. We understand the value of having a trusted companion, especially during the initial visit or when discussing sensitive health matters.
Can I claim the cost of my physiotherapy on private medical insurance?

Do I need to have an X-ray or MRI before I see you?
NO! An X-ray or MRI is not required before any appointment at Centre Street Physiotherapy. If we feel that one is recommended before continuing treatment with us then we will let you know at your initial appointment. If you do have an X-ray or MRI report, feel free to bring the report with you and we will review it with you.
Do I need a prescription from my doctor to be treated?
NO! In the state of WA, patients do NOT need a prescription from a MD to receive physical therapy treatment. If you are planning to file for insurance reimbursement, it is possible your insurance company will require you to obtain a MD prescription. However, one is not necessary to receive treatment from us.
